If you are coming to Evergreen for surgery, you will likely have a pre-admission appointment for testing and blood work. Check with your physician to see if his office handles this scheduling, or if you must schedule it yourself. For some procedures, the pre-admission process can be handled over the phone.
Insurance Approval
You should also contact your insurance company to determine if there is a co-payment amount or restrictions on where you can have a prescription filled. For your convenience, co-payments can be made at Patient Registration. Please bring your insurance card and/or a completed insurance form with you.
